Enrolling in Fallon Health Weinberg-PACE

To fully understand all of your or your loved one's needs and make sure that Fallon Health Weinberg-PACE is a good fit, enrolling in Fallon Health Weinberg-PACE is a multi-step process. The process includes:

  1. Intake
  2. Assessment
  3. Enrollment
  4. Approval

Our intake and enrollment coordinator is happy to answer your questions and to schedule your visit to our PACE Center. 

 

All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
