Information about our plans

To meet the health care needs of dually eligible (receives Medicaid and Medicare) residents in Erie and Niagara Counties, Fallon Health Weinberg offers three health insurance plans:

• Managed Long-Term Care plan (MLTC)
• Program of All-inclusive Care for the Elderly (PACE)  (Erie County only)

The goal of all our plans is to help members stay healthy and continue living safely at home and in the community. No matter which plan they choose, members and their Care Team will create a care plan that is member-centered and individualized. Care plans include services that help each member reach their specific health and wellness goals.
