Fallon Health Weinberg names Craig Dlugosz, RN, as PACE Director

Worcester, Mass., February 8, 2024 – Fallon Health Weinberg is pleased to announce the recent appointment of Craig Dlugosz, RN, as Director of Fallon Health Weinberg-PACE, its Program of All-Inclusive Care for the Elderly (PACE) in Amherst, New York. Dlugosz brings deep clinical and professional experience across a variety of health care settings that support older adults with complex medical needs. In his role as PACE Director, Dlugosz will be responsible for the overall clinical and operational leadership of Fallon Health Weinberg-PACE, as well as the strategic planning, coordination, and evaluation of services provided.

Prior to joining the Fallon Health Weinberg-PACE team, Dlugosz held leadership positions with Fallon Health as well as Schofield Care, Complete Senior Care PACE, and previously worked for a visiting nurses association, all in Western New York. He is a Registered Nurse and a Physical Therapist Assistant. His background, paired with his established relationships with New York State regulatory partners and community partners, solidifies his position as a strong leader for the program.

Fallon Health Weinberg-PACE is an innovative program that helps manage the care needs of older adults so they can remain living as independently as possible at home or in the community while also supporting caregivers. Fallon Health Weinberg-PACE currently serves approximately 173 participants in Erie and Niagara counties.
