Complete care for independent living

Fallon Health Weinberg-PACE is a long-term care program for older adults who need a nursing-home level of care, but wish to stay living at home and in their community. We're a Program of All-Inclusive Care for the Elderly (PACE), providing complete medical care and support services so you or your loved one can remain living safely at home

As a health insurance plan, Fallon Health Weinberg-PACE pays for all of the care and services that participants receive through our program. Learn more ►

All care is coordinated and provided by a Care Team specializing in geriatric care. The Care Team works with participants and their caregivers to create an individualized care plan.

The services provided by Fallon Health Weinberg-PACE (FHW-PACE) may include:

  • Comprehensive medical and specialty care
  • Day center services 
  • Prescription drug coverage
  • Medical transportation  
  • A Care Team of geriatric experts (also called an Interdisciplinary Care Team) 
  • Home care
  • Caregiver support 

Fallon Health Weinberg-PACE may be a good fit for:

  • Caregivers and family members who want to keep their loved one at home as long as possible with supportive services and home care

  • Individuals who want to have all their medical care provided or coordinated by a team consisting of geriatric specialists (Care Team)

  • Individuals who want to engage in recreational and social activities and receive personal care and supervision during the day in an adult day health center

Who is eligible to join Fallon Health Weinberg-PACE?

To be eligible to join Fallon Health Weinberg-PACE you must:

  • Be at least 55 years old
  • Live in Erie County or Niagara County, NY
  • Meet nursing home eligibility criteria in New York
  • Require Fallon Health Weinberg-PACE long-term care services for more than 120 days
  • Be able to live safely in the community, as defined by the New York State Health and Safety Criteria and the PACE Care Team
  • Be Medicaid and/or Medicare eligible or willing to pay privately
  • Meet other eligibility requirements (eligibility addendum)

 

All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
