Fallon Health Weinberg has plans designed to keep members living safely at home and in their communities: a Managed Long Term Care (MLTC) plan, a Medicare Advantage Health Maintenance Organization Special Needs Plan (HMO SNP), and a Program of All-Inclusive Care for the Elderly (PACE).

Fallon Health Weinberg is an HMO plan with a Medicare contract and a coordination of benefits agreement with the New York State Department of Health. Enrollment in Fallon Health Weinberg depends on contract renewal.

All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
