The goal of Fallon Health Weinberg-MLTC (FHW-MLTC) is to keep you living independently in the comfort of your own home by coordinating and authorizing the care, support and services.
When you become a member, your care will be managed and coordinated by a care team of health professionals. They will help arrange all covered and non-covered services you need.
Your long-term care needs will be covered by FHW-MLTC. Non-covered services will be covered by your fee-for-service Medicare and/or Medicaid.
Covered services
These covered services may be a part of your personalized care plan:
- Personalized care management and care coordination
- Home health care
- Dental services
- Non-emergent medical transportation
- Personal care
- Consumer Directed Personal Assistance services(CDPAPs)
- Durable medical equipment (DME)
- Medical/surgical supplies
- Rehabilitation therapies, such as occupational, physical, speech and respiratory, in home or in community
- Prosthetics and orthotics
- Medical social services
- Social/environmental support
- Home-delivered meals
- Adult day health care
- Social day care
- Personal emergency response systems (PERS)
- Eye exams and eye glasses
- Hearing exams and hearing aids
- Podiatry
- Nursing home care

Your Care Team and individualized care plan
Once you enroll in the Fallon Health Weinberg-MLTC plan, your Care Team will work with you and your primary care physician to coordinate all covered and non-covered services that you will need.
Your Care Team arranges physician appointments and necessary transportation, home services, equipment and supplies. When your plan of care includes attendance at an Adult Day Program, we will arrange for transportation to the center and back home.
Your Care Team will continually monitor your health status and will update your plan of care as your health care needs change. Your plan of care will be formally re-evaluated every 180 days.
Through the plan of care and the supports that your Care Team authorizes, you will be able to live safely in the comfort of your own home for as long as possible.
