Fallon Health Weinberg expands services for individuals needing long-term care

Managed Long Term Care services now available for adults age 21 and older in both Erie and Niagara counties

Amherst, NY, February 17, 2015 - Fallon Health Weinberg is expanding its service area and eligibility for its Managed Long Term Care plan (MLTC). Effective today, Fallon Health Weinberg’s services will be available to eligible individuals 21 years and older in both Erie and Niagara counties.

Fallon Health Weinberg MLTC, the former Total Aging in Place program, is designed to help individuals with long-lasting chronic illnesses or disabilities to live safely and independently in their own homes and communities. The program provides and coordinates health care and supportive services to promote and support independence. Services that are covered by Fallon Health Weinberg include home care, non-emergency transportation, adult day programs and more.

Individuals who are 21 and older, live in the service area of Erie or Niagara counties, require long-term care services for more than 120 days and are eligible for Medicaid are invited to join Fallon Health Weinberg.

In addition to the expansion of its MLTC plan, Fallon Health Weinberg offers a Medicare Advantage Special Needs Plan (HMO SNP) for individuals 21 years and older who are eligible for both Medicare and Medicaid. Fallon Health Weinberg has also applied to become a provider of a Program of All-inclusive Care for the Elderly (PACE). PACE is a Medicare and Medicaid program that helps people age 55 and older meet their health care needs in the community instead of going to a nursing home or other care facility.
