Evidence of Coverage

The Evidence of Coverage is a booklet you get when you join Fallon Health Weinberg-HMO SNP. It explains what benefits you have and how to use your benefits. It will also tell you what is covered, how to make a complaint and how to disagree or appeal any decision that is made about your health care. It also tells you about your prescription drug coverage.

You can find information about grievance, coverage determination and appeal processes in your Evidence of Coverage. Please check the section called "What to do if you have a problem or complaint (coverage decisions, appeals, complaints)."

You can also find information about your rights and responsibilities as a member of Fallon Health Weinberg-HMO SNP in your Evidence of Coverage. Please check the section called "Your rights and responsibilities."

Summary of Benefits

The Summary of Benefits is a brief summary of your plan benefits. It does not show you all your benefits, and it doesn't show you the things that your plan does not cover. For a full list of your plan benefits, see your Evidence of Coverage (above).

Annual Notice of Changes (ANOC)

An ANOC is a document which explains any changes in your coverage, costs, or service area that will be effective on January 1.

 

 

Fallon Health Weinberg is an HMO plan with a Medicare contract and a coordination of benefits agreement with the New York State Department of Health. Enrollment in Fallon Health Weinberg depends on contract renewal. This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments, and restrictions may apply. Benefits may change on January 1 of each year.
