Fallon Health Weinberg-HMO SNP covers all your Medicare benefits and coordinates your New York State Medicaid benefits, including medical services, behavioral health services, and medications. This means that as a member, you will need to show your Fallon Health Weinberg-HMO SNP ID card and your New York State Medicaid ID card when you get care. You won't need to show your Medicare ID card.

You should take your Fallon Health Weinberg-HMO SNP and New York State Medicaid ID cards with you to your appointments. You should also take them with you to the pharmacy. It helps your doctors and others understand which plans you are in.
