Fallon Health Weinberg-PACE is a Program of All-Inclusive Care for the Elderly (PACE), serving older adults in Western New York who want to remain living safely at home.

Care is delivered through a dedicated local team of health care professionals who specialize in geriatrics and work closely with participants and their family caregivers to offer complete, coordinated, individualized care and services for older adults who want to live at home instead of a nursing home.

Founded in 1977, Fallon Health is a not-for-profit health care services organization with decades of experience in delivering coordinated care for older adults through Medicare, Medicaid, and PACE programs. That experience supports Fallon Health Weinberg-PACE’s work in New York, ensuring that participants receive high-quality, integrated care that is tailored to their needs.
