Getting care

Once you enroll in the program, you’ll work with a Care Team to identify and evaluate your needs. We will create a comprehensive plan of care that is personalized to help you remain safely at home and in the community. All of your medical and specialty care is coordinated by your Care Team at the Fallon Health Weinberg-PACE Center.

Your Care Team and individualized plan of care

Your Care Team, also called your "Interdisciplinary Team," includes care professionals with geriatric experience. The team may include a primary physician, nurse practitioners, nurses, specialists, social workers, rehabilitation staff, activities coordinators, health aides, home care specialists, a nutritionist and others.

You’ll be cared for by a team who take the time to get to know you and who have access to your care plan and electronic medical record.

The Fallon Health Weinberg-PACE Center

You may receive most of your care (including medical visits, routine lab work, medical treatments and services like physical therapy) on-site at the Fallon Health Weinberg-PACE Center.
