Medication Therapy Management Program

What is the Medication Therapy Management Program (MTMP)?

The MTMP is not a benefit. It's a voluntary program that helps you stay healthy when you are taking multiple drugs. It is available to qualified members at no extra cost.

Who is eligible for the Medication Therapy Management Program?

You qualify for the program if:

  • You are enrolled in a Medicare Part D plan.
  • You are taking at least eight drugs.
  • You have three or more chronic conditions.*
  • The amount your health plan spends on your drugs is more than $3,919 annually.

* Qualifying chronic conditions include: high blood pressure, high cholesterol, diabetes, COPD (chronic obstructive pulmonary disease), asthma and depression.

If you qualify, you will get a letter from us. This letter will explain the program and let you know that you are enrolled in the program. After you receive the letter, a member of the Medication Therapy Management team will contact you by phone, unless you call the number on the letter saying that you don’t want to participate (“opt out”).

What do I receive as a MTMP participant?

You get at least one phone meeting with the clinical team to go through a review of your medications. You also get quarterly drug reviews.

The phone meeting takes less than 20 minutes. A Care Team member gets the information to do a full drug review. This drug review does not hinder the care by your doctors. It may include looking at:

  • How your drugs interact
  • Ways to help reduce side effects
  • Lower cost alternate drugs
  • Simplifying your drug intake

You get a summary of the review. You also get a list of suggestions from the pharmacist.

You can also call us to get copies of letters that we have sent you, to get blank medication lists or to get educational materials. You may also print a blank medication list (pdf). The blank medication list is to help you keep track of the medications you are taking.
